Fracture of bones is commonly handled by almost all orthopaedic surgeons in India. But dealing with comminuted intra-articular fractures (involving joints) is a night mare for most of them. It requires special expertise and experience to deal with such kind fractures. It can involve pelvic acetabular fracture, upper portion of thigh bone ( hip), lower portion of thigh bone ( femur), upper portion of arm bone (humerus head), lower portion of arm bone (humerus), lower portion of forarm bone (lower end of radius) fracture of heel bone, etc. The idea is to maintain the joint (articular) integrity and prevent the permanent loss of movements at that joint. It requires image intensifier (c-arm) to do these surgeries. May be artificial bone or autograft (patient’s own bone taken from iliac bone) to fill in the bone defect/ loss if present. The fracture may take 2- 3 months to heal. Patient may require prolonged physiotherapy to gain near normal range of motion exercises.
